Survey Questions

This organization receives funding from the Ohio Attorney General. Your anonymous responses help improve services available to crime victims in Ohio.

For each question, select one response: Strongly Agree, Agree, Neutral, Disagree, Strongly Disagree, or Not Applicable.

1. As a result of working with this program, I understand my rights as a crime victim.
2. I was assisted in meeting my immediate needs: mentally or emotionally.
3. I was assisted in meeting my immediate needs: physically, including safety.
